What is HER2?

HER2 stands for Human Epidermal growth factor Receptor 2. It is a protein found on the surface of all breast cells, playing a vital role in their normal growth, division, and repair. In healthy cells, HER2 acts like an antenna, receiving signals that tell the cell when to grow and divide.

However, in some cancers, particularly breast cancer, there can be too many copies of the HER2 gene or an overproduction of the HER2 protein. This excess HER2 signaling can lead to uncontrolled cell growth and division, making the cancer more aggressive. When this occurs, the cancer is referred to as "HER2-positive."

What Does "HER2-Negative" Mean?

When a breast cancer is described as "HER2-negative," it means that the cancer cells do not have an excess of the HER2 protein on their surface. In other words, the HER2 gene is not amplified, and the protein is not overexpressed. This is a common finding, as approximately 70-80% of all breast cancers are HER2-negative.

This designation is crucial because it indicates that the cancer is not primarily driven by overactive HER2 signaling. Consequently, specific targeted therapies designed to block the HER2 protein will not be effective against this type of cancer. How is HER2 Status Determined?

After a breast cancer biopsy or surgery, tissue samples are sent to a pathology lab for detailed analysis. Pathologists use specific tests to determine the HER2 status of the cancer cells. The two primary tests are:

Immunohistochemistry (IHC)

IHC is often the first test performed. It measures the amount of HER2 protein on the surface of the cancer cells. Results are typically reported on a scale from 0 to 3+:


  • 0 or 1+: These scores usually indicate HER2-negative status, meaning there is little to no HER2 protein detected.

  • 2+: This is considered an equivocal result, meaning the amount of HER2 protein is borderline. Further testing is needed.

  • 3+: This score indicates HER2-positive status, meaning there is a significant overexpression of the HER2 protein.

Fluorescence In Situ Hybridization (FISH) or Dual-ISH

If the IHC result is equivocal (2+), a FISH or Dual-ISH test is performed. These tests look for extra copies of the HER2 gene within the cancer cells themselves, rather than just the protein on the surface. If these tests show no amplification of the HER2 gene, the cancer is confirmed as HER2-negative.

Why is HER2 Status Important for Treatment?

Knowing the HER2 status is fundamental for oncologists to plan the most effective treatment strategy. This is because HER2-positive cancers respond well to a class of targeted drugs specifically designed to block the HER2 protein (anti-HER2 therapies). Conversely, anti-HER2 therapies are not effective for HER2-negative cancers because the HER2 protein is not driving their growth. Therefore, a HER2-negative diagnosis guides treatment away from these specific targeted drugs and towards other well-established and effective options. Treatment Approaches for HER2-Negative Breast Cancer

Since HER2-negative cancers do not respond to anti-HER2 targeted therapies, treatment plans are tailored based on other crucial factors. These include the cancer's hormone receptor status (whether it's estrogen receptor-positive and/or progesterone receptor-positive), tumor size, lymph node involvement, grade of the tumor, and the patient's overall health and preferences.

Chemotherapy

Chemotherapy is a common and often highly effective treatment for many HER2-negative breast cancers. It uses powerful drugs to kill fast-growing cancer cells throughout the body. Chemotherapy may be given before surgery (neoadjuvant), after surgery (adjuvant), or for advanced disease.

Hormone Therapy

If the HER2-negative cancer is also hormone receptor-positive (meaning it grows in response to hormones like estrogen or progesterone), hormone therapy (also called endocrine therapy) may be a key part of the treatment plan. These drugs work by blocking hormones from fueling cancer cell growth or by lowering hormone levels in the body.

Surgery and Radiation

Local treatments like surgery (lumpectomy or mastectomy) and radiation therapy are often crucial components for both HER2-negative and HER2-positive breast cancers. Surgery aims to remove the tumor and potentially nearby lymph nodes, while radiation therapy uses high-energy rays to destroy any remaining cancer cells in the breast and surrounding areas.
